MOBILE PASSPORT PROCESSING SERVICES

In partnership with the Department of Foreign Affairs, the City Government of Catbalogan will be conducting a Mobile Passport Service this August 17, 2013 at the City Plaza. The requirements are the following:

1. FOR NEW APPLICANTS (18 years old and above):
a. Birth Certificate (BC) in Security Paper issued by the National Statistics Office (NSO) / Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) duly authenticated by the NSO with certificate of Non-availability of Record from NSO
b. For married women – Marriage Contract (MC) in Security Paper issued by the NSO/ Commission of Filipino Overseas (CFO) Guidance and Counselling Certificate of Attendance (for spouse of foreign nationality)

– if aforementioned documents are blurred, CTC of BC or MC from the Civil Registrar’s Office is required

c. at least 2 valid Identification Cards, example:
– PRC ID/ IBP ID
– Digitized SSS ID
– Driver’s License
– Company Office ID
– School ID
– GSIS E-Card
– Seaman’s Book
– OWWA ID
– Voter’s ID
– Senior Citizen’s ID
– Digitized BIR/TIN ID
– other acceptable IDs such as Alumni ID or any old ID
d. for applicants with late-registered birth certificates – supporting documents with complete and correct full name, date and place of birth,

Example:
– voter’s registration record & voting record w/ COMELEC seal and official receipt
– School Record (Form 137 A and Form 137 E with school seal)
– Government Service Record
other documents may be required when necessary esp. for applicants with dual citizenship or foreign sounding family names to prove citizenship

2. FOR APPLICANTS BORN BEFORE 1950 WITH NO BIRTH RECORD
a. Certificate of No Birth Record issued by NSO
b. Certificate of No Birth Record issued by LCR
c. Joint Affidavit of Two Disinterested Persons
d. any public document with correct place and date of birth, example:

– voter’s registration record & voting record w/ COMELEC seal and official receipt
– School Record (Form 137 A and Form 137 E with school seal)
– Government Service Record
– Baptismal Certificate
other documents may be required when necessary

3. FOR MINOR APPLICANTS (both new and renewal of passports):
a. Birth Certificate from NSO
b. Personal appearance of applicant
c. Personal appearance of either parent/s, Marriage Contract & valid IDs of parents (if child is legitimate)
d. Passport of travelling companion
e. other documents may be required when necessary
f. additional requirements if child is not travelling with parents:
– DSWD Clearance
– Affidavit of Support or Consent from both parents indicating the name of the travelling companion
– at least 2 valid ID cards of parents/ authorized person
– if both parents are abroad, the Affidavit of Support and Consent must be authenticated by the nearest Philippine Embassy or Consulate (indicating the name of travelling companion). Parents must also execute a Special Power of Attorney also authenticated by Philippine Embassy, indicating the person they designate to act in their behalf in securing the passport of the minor.

4. FOR RENEWAL:
a. Green Passport
– personal appearance
– original and photocopy of passport, first and last page
– original and photocopy of ID Card

b. Brown Passport
– personal appearance
– original/photocopy of passport pages 1 – 5
– birth certificate from NSO
– at least 2 valid ID cards

c. Maroon Passport
– personal appearance
– original and photocopy of passport, first and last page
– Birth Certificate from NSO/old passport

5. REPLACEMENT OF LOST PASSPORTS:
a. valid passports
– documents required for first-time passport applicants
– affidavit of loss
– police report
– subject to 15 working days clearance

b. expired passports
– documents required for first-time passport applicants
– affidavit of loss
– subject to 6 working days clearing period

20 working days – PhP 1,200
35 working days – PhP 950
